Construct a Logic Model

ABOUT THIS WORKSHEET:
There are many examples of logic models and theory of change worksheets and courses online. The most basic form of logic model is a roadmap for implementation that systematically connects initial resources with desired outcomes

TIPS:
  • Activities. In order to address our problem or create this asset, we will conduct the following activities.
  • Resources. In order to accomplish our planned activities, we will need the following resources. 
  • Outputs. Once completed or under way, the activities will produce the following evidence of service delivery.
  • Short- and long-term outcomes. We expect that, if completed or ongoing, these activities will lead to the following changes in one to three, then four to six, years.
  • Impact. We expect that, if completed, these activities will lead to the following changes in seven to ten years.
